Initial Registration

Shipping Locations

All locations managed within this portal must be pre-loaded into the site. Upon initial registration, your account will have access to ship the location identified on the registration form. If you would like additional locations loaded into your account that can be used as ship-to locations, please complete and return the shipping locations spreadsheet and return it with your initial registrations.

Payment Authorization

All T-Mobile Authorized Retailers / Non-Retail are initially granted the payment option of a credit card. To request the ability to pay using a purchase order and ACH, please download, complete and sign the authorization form below and return it with your T-Mobile Dealer / Non-Retail Registration Form.
